For example, you may be seeking patient and family representatives for an advisory group, steering committee, priority-setting activity, co-design process, or other involvement activities.

Please complete the form below. To help us identify suitable representatives, it is important to clearly describe the purpose of the project, the expected role of participants, and how they will be involved.
